影響自閉症類群者溝通表現之前事因素及處理策略

研究問題：近年來我國自閉症類群(ASD)人口數逐漸增加。社交溝通及社會互動上的缺損為 ASD 者共同之困難，即使是能力較好的 ASD 者，仍會出現不同類型之互動缺損。研究者在自身身上觀察到一些因素會阻礙 ASD 者與人互動，而企圖以本研究找出阻礙其發揮原先最佳狀態溝通表現的前事因素，進而有機會預防或發展出對應的處理策略。研究對象：本研究之研究對象為有能力填寫本研究問卷之 ASD 本人或 ASD 者之主要照顧者，有 19 位 ASD 之主要照顧者及 7 位 ASD 者共 26 位受試者參與本研究。本研究所探討之 ASD 樣本指經身心科醫師鑑定為 ASD，或領有 ICD 碼為「F84」之身心障礙
證明者。研究參與者主要來自閉症家長團體、臺北市自閉兒社會福利基金會、早療日托機構學生家長。研究方法：本研究採用質性問卷方式，蒐集受試者溝通表現下降經驗之及因應方式等訊息，以了解所探討之 ASD 樣本在何種狀態或前事因素下出現負向溝通表現之改變以及有效的處理策略，並請受訪者以李克特氏五點量表對研究結果之符合程度做評分，做本研究之三角驗證。研究結果: 研究結果找出對流暢性有影響的前事因素 11 項，其中負面情緒達高度影響。主動溝通 22 項，其中受責備與負面情緒達中度影響以上。口語接收理解 17 項，其中負面情緒:生氣/難過、對話內容複雜度、不理解對話內容、受責備以及突然不舒服的感覺刺激達高度影響
。仿說頻率 6 項，其中不理解對話內容、負面情緒:生氣/難過、受責備、對話內容複雜度以及「突然不舒服的感覺刺激」達中度以上影響。思考速度 22 項，其中「不理解對話內容」、「對話內容複雜度」以及「負面情緒:生氣/難過」達高度影響。整體來看「負面情緒:生氣/難過」、「受責備」、「壓力」「突然不舒服的感覺刺激」、「不理解對話內容」、「對話內容複雜度」、「挫折」、「語言」以及「專注力不足」等因素影響整體溝通表現。進一步歸納分類，發現情緒、身心狀態及感官刺激是影響整體溝通表現的三大類因素，對所探討的溝通表現都有很大的影響。在溝通表現受影響之時，「以點頭/搖頭回應」方式為接受度最高的溝通方式，其次為筆談
及電腦打字溝通，而恢復策略方面則由質、量性資料整理出 21 個有效恢復溝通表現的策略。結論與建議:建議 ASD 者及其照顧者，當突然發生溝通表現下降時，應先檢視情緒、身心狀態及感官刺激等三項因素，若仍不清楚影響其溝通的前事因素為何，建議條件允許的情形下可優先以筆談或打字的方式了解原因，與其溝通。若 ASD者無法接受在此情況下有任何溝通行為，則可嘗試本研究證實有效之恢復策略，若能知道其因素，則更可以因應不同類別之前事因素加以處理，每一類別均提供4-5 項對應之有效處理策略供 ASD 之照顧者參考。
