背景:職業噪音性聽損為國內常見之職業病。警消人員健康檢查雖包含聽力檢查,卻無量測資料說明其噪音暴露危害情形及與聽力之相關性。目的:針對警消義交進行職場噪音量測、檢視其聽力狀況,並將兩者作分析。方法:於2020年9月開始、為期一年的聽力資料收集(純音聽力與耳聲傳射)和職場噪音量測(同時收錄3種模式),並進行健康狀態與聽覺問卷調查。受試者為36名警消義交和36名醫院員工(為對照組),比較兩組噪音與聽力之差異。使用單變量分析單一音頻耳聲傳射與純音聽力之相關強度、繪製接受者操作特性曲線(receiver operating characteristic, ROC curve)判斷耳聲傳射預測該頻率聽

損與否之準確性、使用多變量分析(典型相關與多元迴歸分析)將單一刺激音頻耳聲傳射對純音聽力(世界衛生組織標準、中音頻率、高音頻率)預測之角色與權重作定調。結果:兩組聽損程度多處於正常範圍、警消義交組於劣耳高音頻率聽閾(3000、4000、6000赫茲平均值)有6人(16.7%)超過25分貝;4人輕度、2人中度聽損。除左耳中音頻率外、兩組聽閾於各耳各頻率區皆有顯著差異。兩組職場噪音劑量中位數於各種標準皆有顯著差異。使用耳聲傳射訊號強度建立職業噪音聽損模型,以右耳2000、3000、5000赫茲和左耳3000、6000赫茲較具模型解釋力。結論:本研究警消義交於工作期間所進行之個人噪音量測,顯示無職場

噪音危害,但仍需考量個人輪班、任務型態之差異。變頻產物耳聲傳射預測聽力準確度高、亦為噪音性聽力損失模型中重要之解釋變數;建議具有聽損風險的行業、於職前健康檢查時作為勞工聽力相關評估工具。
