研究背景：高血壓是引發認知障礙的重要危險因子，且透過擴散張量磁振造影 (diffusion tensor imaging, DTI) 已證明高血壓對於白質微結構完整性造成影響。然而，DTI無法有效處理複雜神經交錯或分支情形，難以描繪出部分神經纖維的走向和變化。通用擴散波數取樣磁振造影 (generalized q-sampling MRI, GQI) 能夠突破DTI的限制，但目前尚無研究運用GQI來探討高血壓或是腦血管因子對白質微結構之影響、及其與高血壓相關認知功能缺損之相關性。研究目的：本研究目的有三：（一）探討高血壓對認知功能影響；（二）運用擴散波數取樣磁振造影，比較高血壓族群與一般健康

人的大腦白質微結構差異；及（三）探討高血壓族群認知功能與大腦白質微結構的關係。研究方法：本研究納入50歲以上高血壓患者85位，以及健康成人57位作為控制組，兩組皆接受完整認知功能評估，並以巢狀設計安排部分受試者接受神經影像檢查（高血壓組28位；控制組27位），利用通用擴散波數取樣磁振造影與基於體素之統計分析 (voxel-based statistical analysis, VBA)、獨立樣本t檢定與多元回歸分析進行分析。研究結果：相較於控制組，高血壓組在數字符號替代測驗、路徑描繪測驗A版、長庚大學字型流利度測驗表現顯著較差，且在中文版語言學習記憶測驗中的立即回憶總數有較少的趨勢。影像結果顯

示，高血壓組的白質微結構完整性相較於健康成人組有所變化，包含額葉、頂葉、枕葉、胼胝體膝部及下額枕束。而多元回歸分析結果顯示，高血壓族群的執行功能與額葉、顳葉、下側頂葉區、中央前區、中央後區及前側冠狀輻射的白質微結構完整性達顯著相關；記憶功能與中央後區及下側頂葉區的白質微結構完整性達顯著相關；視空間功能與上側顳葉、胼胝體壓部及視丘的白質微結構完整性達顯著相關；語言功能與上側冠狀輻射的白質微結構完整性達顯著相關。研究結論：高血壓族群較健康人相對有較差的執行功能、處理速度、立即回憶表現，白質微結構完整性出現變化，且其認知功能與白質微結構完整性有關。透過GQI，除了驗證過去相關研究的結果，也發現除了過

去已知神經束之外和高血壓相關的腦部病變位置。
