背景與研究目的:步行障礙是中風個案常見的動作功能問題,而個案常見之步態問 題包含患側腳步長縮短與患側單腳站立時間減少。近期跑步機結合生物回饋訓練已 被用於改善中風個案步態表現,本研究參考先前研究提供之生物回饋方式進行調 整,分別藉由提供「時間步態參數」、「空間步態參數」或同時提供「時間與空間步 態參數」之回饋介入,以探討跑步機合併不同類型之生物回饋訓練對於慢性中風個 案的步長與單腳站立時間之立即性效果。 方法:本研究為單盲隨機對照試驗,參與者會隨機分配到跑步機訓練組、跑步機合 併視覺回饋組、跑步機合併聽覺回饋組和跑步機合併視覺與聽覺回饋組,進行單次 40 分鐘訓練。跑步機合併視覺回饋組會將不

同顏色的膠帶貼在跑步機跑帶旁邊作為 每步踩踏之目標線,並在個案面前放置運動攝影機與螢幕以提供行走時之視覺回 饋;跑步機合併聽覺回饋組會讓個案穿上裝有壓力感測器的鞋墊來偵測行走過程中 單腳站立時間,並使用聲音作為行走時之聽覺回饋;跑步機合併視覺與聽覺回饋組 則為同時採用前兩種回饋方式。本研究分別在介入前與介入後進行結果測量,主要 測量項目為步長與單腳站立時間,次要測量項目包括空間與時間步態參數、空間與 時間步態對稱比率與功能性測試等。 結果:跑步機合併視覺回饋組在介入後兩腳步長有顯著進步,且患側腳步長之改變 量分別與其他三組有組間差異。跑步機合併視覺與聽覺回饋組在介入後患側單腳站 立時間與健側腳

擺盪時間有顯著增加;對照組在介入後健側單腳站立時間與患側腳 擺盪時間有顯著減低;且此兩項參數的改變量分別與跑步機合併聽覺回饋組與跑步 機合併視覺與聽覺回饋組有組間差異。四組在行走速度、步頻、空間與時間步態對 稱比率並無組內與組間之顯著變化。功能性評估中跑步機合併視覺回饋組在計時起 走測試有組內進步,對照組在六分鐘行走測試有組內進步。 結論:跑步機合併視覺回饋訓練對於慢性中風個案患側腳步長有立即性的效果,且其效果優於純跑步機訓練。此外,本研究所使用之聽覺回饋鞋墊應在日後進行多次 介入,以驗證其對於慢性中風個案患側單腳站立時間與相關步態表現之效果。
